GeriNet is a regional network of health care professionals with an established reputation for providing quality and cost-effective primary medical services to nursing home residents. Our vision is to improve the quality of life for long-term care residents by revolutionizing the delivery of health care. GeriNet contracts with health plans, PPOs, medical groups and IPAs on a capitated and fee-for-service basis to provide specialty physician services. GeriNet has developed a delivery model that improves patient care, reduces overall system costs and relieves the Primary Care Physician of the time-consuming responsibility of in-patient nursing facility rounding. GeriNet currently services more than 360 skilled nursing facilities in San Diego, Orange and Los Angeles counties in Southern California as well as in Las Vegas, Nevada.

GeriNet assists Primary Care Physicians in managing the post-acute population allowing the PCP to maintain a productive clinic patient caseload. As an extension of PCP, GeriNet physicians provide patients with the highest level of care in the post-acute and long-term care setting and maintain efficient communication with the PCP. Our case management systems support a superior level of utilization management and coordinated discharge planning.
